Job Description
- With the guidance of a senior staff, provide quality care to patients through clinical assessments, effective communication and active participation within a multidisciplinary team environment.
- Provide a comprehensive assessment to enable a specific intervention plan to be developed in consultation with the patient and their family. Coordinate the referral and evaluation of patients to across diagnostic teams and other services communicating social work plans to medical and nursing staff, documenting all aspects of social care following established protocols, policies and procedures.
- Coordinate and counsel patients and their families in cases of difficult reaction to illness, disabilities, death and handicap and assist patients in adjusting to illness or disability and support family members in understanding the patient's needs.
- Counsel and advice patients that refuse admission or patients seeking to discharge from the hospital against medical advice.
- Assist the Health Care Team involved in patient's treatment plan by solving social problems such as work related problems, marital problems and other problems that may interfere with the care of the patient.
- Coordinate and participate in the discharge planning services which include information, referral and community liaison services.
- Assist with implementation and evaluation of protocols and procedures to ensure quality care for patients.
- Assists in discharging long-term patients either through their home or alternatively appropriate facility, ensuring patients obtain necessary medical equipment, transportation and post-hospital care.
- Assist in making an accurate assessment of the post-hospital care that the patient will require.
- Provide a liaison, a good relationship and help to improve communication between patients and hospital staff.
- Contribute to relevant committees requiring involvement of Social Services as assigned and participate in patient rounds on a daily basis.
- Participate in orientation programs and attend continued education and training from the University.
